Supervisory Program Analyst

Mrs. Hernandez is a Supervisory Program Analyst within the Research and Development team of National Care Coordination (NCC), Veteran’s Crisis Line (VCL). In this role, she is responsible for overseeing the implementation and evaluation of new pilot programs, evaluating various program aspects to ensure effectiveness of services provided and determining areas of improvement. Mrs. Hernandez is also a Licensed Clinical Social Worker.
Prior to joining this team, Mrs. Hernandez served as a National Clinical Specialist with the National Advance Care Planning via Group Visits (ACP-GV) Program where she worked on the daily operation of implementing an innovative best practice across the enterprise and providing leadership for national education, training, and consultation for the field.
Her previous roles within the VA include direct practice as a Senior Mental Health Social Worker, and care management within the Transition & Care Management program (TCM) and Patient Aligned Care Team program (PACT).
Before joining VA, Ms. Hernandez worked as an independent therapist at a group practice and as an advocate for high risk populations.
